There are so many accounts out there that you can get into with "password". Or admin, administrator, 123456, 1q2w3e4r, etc etc. It's horrible.

Reminds me of back in the mainframe days when a friend of mine relayed how he got "interviewed" by the sysadmin and his manager. This was shortly after some giant scandal where a govt or contractor system got hacked into and they forced everyone to endure a big time lecture about company policies regarding how passwords are super double dawg secret, never write them down, tell another soul, yadda, yadda followed by instruction for everyone to immediately change their passwords, which he dutifully did, along with the rest of us. Being under the gun on a project, and slightly pissed at the time wasted in the middle of the afternoon, he changed his to "fuckyou1". The next day he got called into the office for the "interview", where the sysadmin and his manager chewed his butt for a while about inappropriate language, etc. He looked at them and asked how they knew what his password was. The answer was that the sysadmin had access to the password file, at which point my friend told them he wanted to report a security violation by the sysadmin, and the manager should turn himself in for not turning in the sysadmin, in accordance with company policy that had been stressed the previous day. Needless to say, the matter was dropped with alacrity.

Sounds like social engineering, more than any hack. A technical con if you will.
I will say you shouldn't be surprised at the amount of "default" passwords that aren't changed. It was a great way to gain access to routers. (admin:admin) Now, most routers require a new password to start the setup.
Friendly PSA: No one needs to admit it, but if your password is in a dictionary, it's not secure. A lot of people create passwords related to your birthday, anniversary, a sports team or a pet. If I can find the answer on FaceBook (including your security questions) it's not secure. A Passphrase is best. e.g. I am looking at a book "Resistance to Tyranny" by Joseph P Martino, PhD. You could make a password by taking the first letter of each; RtTJpMPhD1! <--You can remember that, but it isn't guessable.

I used to use TrustNo1 but even that became too popular. Biggest problem I have is that they (work mostly) is always asking us to change it. It's getting to be a pain to try and remember what the new one is all the time.

Frequent password changes are one of the biggest security holes in the world.
That and forcing someone to have too many different ones.
The more likely you make it for someone to forget their password the more often it will happen.
The more often your people need to reset passwords, the easier the procedure will be.
How secure is a system that requires a 10 digit password with 4 character classes, but has 100s of them reset every day with a phone call ?

Another thing to do periodically, perform a google search for your password. There are places out there publicly posting hacked password lists and if a site you used a password on was hacked, it may find it's way to one of those public hacked password lists.
