At first the impulse is to say "What an Idiot !"

But then, I have to ask....

What in the world stopped that first round ?

And if you were firing at a distant target, how would you even know that round never left the chamber ?

As far as the cause, all I can think of is a batch of cartridges with every light powder loads.

Perhaps a Re-loader with faulty equipment - bad specs - or just not paying attention.

They must have been very light loads. I saw a guy at my club put 2 into his S&W 686 while shooting bullseye. They were under powered hand loads. He had to take the gun to a Smith to knock them out but the gun was fine afterwards.

Reminds me of guns recovered at Gettysburg, yes thats plural, guns. Apparently the soldiers loaded and fired, not realizing the gun never discharged they kept loading and firing til the barrel was full to the top with a laminate of powder and ball. The adrenalin, confusion and carnage must have been horrific. These were recovered guns, it is assumed their bearers met their end.

Complete lack of recoil would be an indicator the bullets aren't leaving the barrel. stated above blow back should be noticeable. One way to load up a barrel like that is a hammer and rod.

i was a little tot at the time with family at Gettysburg and i remember from the tours of that .. muskets loaded with many charges

Staged would be my guess. Someone decided to have some fun, fired many rounds of 9mm with primer only and with barrel blocked at muzzle. Note the deformity of the round at the muzzle end. Either it knocked out the first round that had stuck or someone purposefully blocked the muzzle and whatever they blocked it with caused first round to deform. Just my guess but you would have to be an absolute moron to get 22 rounds stuck in the barrel with realizing it until the last one.
