At first the impulse is to say "What an Idiot !"
But then, I have to ask....
What in the world stopped that first round ?
And if you were firing at a distant target, how would you even know that round never left the chamber ?
As far as the cause, all I can think of is a batch of cartridges with every light powder loads.
Perhaps a Re-loader with faulty equipment - bad specs - or just not paying attention.
