Extractor fell off

So I'm shooting my 9mm carbine, after twenty rounds I see this sitting under the gun.

After an audible WTF, I determined it to be the extractor, loaded a couple loose rounds in a magazine and fired them off, no problem. The pin was still in the bolt, and good thing because had either of those parts ended up in the ground they are gone forever. My home range is covered in old leaves.
It went right back together but I don't see any mechanism that would stop this from happening again. Have any of you encountered this problem? How did you remedy it?

Reloads or factory ammo?

Regardless, are you able to retrieve any spent brass? Ran into this on a friends carbine years ago. Ammo in handguns just fine, case heads separated in the carbine. Blew out the extractor just like your picture. We thought it was bad reloads but we worked the ammo data backwards and forwards, dissected and analyzed the ammo and found it was within spec. No signs of high pressure when shooting in handguns.

Is your chamber polished and clean? Any chance the brass was hanging up or delayed @ firing? Maybe too much resistance in the chamber? Chamber too tight?

We've resorted to running only new factory ammo through his carbine, making sure the chamber is squeaky clean.

Isn't that retained in bolt with a rollpin? Don"t know how it could have come out if pin was still in bolt are you sure it was secured properly to begin with? Any hairline cracks around the rollpin holes that possibly opened up to allow it to fall out? Does it have correct size pin?

This was brand new S&B ammo, I don't reload for the 9. Nothing is broken or looks forced. Like I said, the pin that holds the extractor in was still in the bolt. I some how shimmied up enough to allow the the extractor to leave. I will go back out and look for casings, it'll be tough finding any though.

Hmmmmm, Not sure C. The bolt came complete as is. I am going to look into a roll pin though, looks like a standard 3/32" hole.
Just went out and found five casings. they look okay to me, very small extractor mark on a couple but certainly not excessive.

Like stated Mek, the pin was still in the bolt, and is solid, not a roll pin. It shimmied up, not down, so it defied gravity.

Then change it for a roll pin like originally intended for the original AR extractor. That way you can flare it up and make
sure stays in place. It is not rocket science.

Does it have an o-ring like a standard extractor. It's not in your picture.

Its a standard 9mm blowback design bolt. I don't remember where I got it, there was a flurry of parts ordering at the time. I will get and insert a roll pin, never thought of it as 'rocket science' Mek. I'm just barely smart enough to know that I don't know everything there is to know about, about, well, any given subject and simply asked for feed back in case there was some nuance in the construction of this particular variation of an AR that I might be missing.
PutnamCharlie, I don't see any evidence of an O-ring, not to say there shouldn't be one, it may have left the room when the part fell off. I'll have to check it out.

Grab a magnet and sweep the area

Robin. Stick to flintlocks. No extractor needed.
