A lot of people dont put to much thought into their carry ammo, some think they do and grab something designed for a 4" gun and put it in their 2" pocket gun.
Here is a video of a officer struggling with someone he is trying to arrest. And finally at one point there is a struggle for the officers gun and the officer fired a round. The round strikes the perp in the forearm. This was point blank, it even induced a malfunction.. the bigger issue it did not "stop" the threat other than the individual running off with a bullet in their arm.
Barriers are a thing in the real world, so are angles, good quality ammo with good penetration matched with the correct gun and barrel
