I can't imagine any law enforcement agency will sign that form. It asks them to certify things they can't possibly know. If Oneida County's position is "get that specific form signed or we will revoke your permit" you need a good 2A lawyer. Around here Amy Bellantoni has the best reputation...

The form is outrageous. Oneida County is asking Florida law enforcement to certify that you have not committed any crime. How would Florida know what you've done? At most Florida LE knows whether or not you were arrested for a crime.
