Just go and get 110lb premium card stock. Cut it into a rectangle and do your finger prints from a black stamp pad....

When I applied for a UPIN (unique personal identification number) the FBI sent a packet including the fingerprint card used by Agencies across the Country (at least until most went digital scans) which are also fully acceptable to submit. Lot of Departments charge a small fee and it's common for many Trust/Sensitive Employers to require fingerprints of their (perspective) employees... Be it your Village Cop/local PD, Sheriff's Office, or even the State Police, somebody should be able to do the task for little or no cost and supply the necessary materials for it.

just get a sheet of white poster board and cut to size and print on that. or I believe you can even get it in same size as printer paper

Do it right. Printer paper won't hold as well and if the 10 card can't be read you'll be denied again.

Most agencies use livescan instead of print cards now. Cleaner and the computer gives you instant feedback if the print is good.

As far as I'm aware, the only time ink is used is for unidentified DOA's, and those are done by the Medical Examiner

I was going to recommend the state police. Sorry for your troubles and it’s ridiculous that you have to go through this. Unfortunately it’s not your FFL’s fault as NYS made the law and he has to follow it.
About the delay just be aware that any delays or denials are now reported to your local
PD. Sounds like your legal and all but just be aware of that b

FWIW, I have been through this shit appealing NICS denials many, many times. And I mean many. Until I received my UPIN, anyways. Go to your local SP barracks and they’ll fingerprint you for free on the FBI card that you printed out from the website. It’s fine, did I mention I’ve done it many times? If there is no one at the barracks , call the phone number listed for that barrack and it’ll go to dispatch if no one answers. Tell them exactly what you’re looking for and they’ll bring an officer in off the road to assist you. This is what I’ve done no less than 8-10 times in a 2 year period. Happened so often that some of the women troopers would say to me, “what are you buying now Mr. REF?” And the fingerprint card on the website you printed out on normal paper was never an issue for me. Usually had my appeal processed within 7-10 days. Think I did the last couple of appeals online and it’s rather streamlined now, my 2 cents anyways

I mailed them in and I actually paid for the expedited delivery because I’m extremely inpatient and when I purchase something, I want it now. Hate waiting on anything.

ORI is the Police Agency number. Each one has an individual number which is not duplicated but goes on every incident report/summons.

OCA is also agency specific. Stands for "originating case agency"

Those boxes are for the agency, (police ,etc) doing your fingerprinting. Leave them blank

When I applied for CT non-res permit, my prints done by a NYS trooper at the barracks were rejected by CT. At the time, I thought some were not clear but wasn’t going to criticize the trooper’s efforts. Later, after I had submitted my application packet to CT, thr rejection email arrived.
Had to go to Middletown CT for them to take prints. They were very meticulous in processing the digital prints. But in the end, they gave me a 2nd FBI card with my prints to submit to Florida for my non-res permit there which had long expired. So you could use that 2nd card to apply for Florida also (provides right to carry in a number of additional states)
