How do you keep range notes

Use a notebook, loose leaf binder or index cards? I use one of those black and white composition notebooks that I coopted from my weight lifting days and my notes are terrible. Almost like some secret code that I developed then forgot.

Example below. If you could tell me wtf this means.

So in order to avoid that in the future. Try this:
Date:
Weather conditions:
Gun:
Load:
Number of rounds fired:

Elevation:
Zero range: example 100
200 yards + 14 from zero
300 yards + 32 from zero
400 yards + 59 from zero

Windage 10mph full value wind
Zero range: + 10 from zero
200 yards + 15 from zero
300 yards + 27 from zero
400 yards + 48 from zero

I figured this out shoot 100 yards 5 times and you just shot 500 yards easy Right
