ok, so it was a hard holster not likely to fold... so fabric of some sort must have gotten in there.

This is one reason I went with the Springfield and the grip safety. I don’t need anymore holes in my body. I know I’m careful, but mistakes happen. I do not engage thebgeip safety while holstering the gun. I just press it in with my thumb on the back of the slide.

That's a good idea.

On my PPS when I re-holster it (while visually looking and taking my time) I keep my thumb on the rear cocking indicator. Of the trigger is being pulled back the cocking indicator begins to protrude out the back of the slide so if the trigger is being snagged in clothing or something I can't see, I can also feel with my thumb the trigger beginning to be caught on something. Kinda the same idea to guys with hammer fired guns keeping thumb on the hammer.
