A: Out of state residents who do not possess either a New York State Driver License or Non-Driver Identification Card must use the paper form to recertify and must attach a copy of their driver’s license or non-driver identification from their state of issuance.

One of the questions I remember was have you ever been denied a carry license or ever had one suspended or revoked for any reason.
As someone here earlier posted, NY is not Shall Issue. If you were to get it revoked and had to have your pistols temporarily held by them until you got it back or even by an FFL, you lose your ability to care for them properly until you get them back. That's if you get them back. No one is going to oil them for you or make sure they don't get a scratch.

The statement by the NYSP is really a bunch of bullshit. Here's why:
They are STILL going to turn over the lists of who has and hasn't recertified to the counties. That means that the counties will know if you have or have not recertified. There is nothing preventing the judges (remember, the 'issuing authority') from proceeding with a revocation hearing and having all your guns seized. And if it comes down as a result of a court order, there is absolutely NO discretion on part of law-enforcement.
They used a whole lot of words to say absolutely nothing.
