With a plus prescription optimized for my pistol gripped at arm distance so my sights are in focus yet the rest of the lens allows normal distance vision. Just got these made at Lenses - Tactical Rx on Spy Bounty frames. Took them out and wasn't sure if my brain would make the adjustment so I could concentrate on my sights but still have a sense of the target. I switched back to my regular "both" bifocals to make sure but yes, it clicked in and was very successful.
Perhaps not for everybody - some people can't adapt - and not cheap (but they only had to grind one lens not two so that saved a bit). It worked out well for me, they are a lot sturdier and better fitting than the generic $15 bifocal shooting glasses of suspect origin.
There are other vendors who do this as well but I don't think you'll find a custom solution for much less. These guys counsel you over the phone, suggest a few frames based on your size and preferences, send you the frames with clear glass to try on and mark up. You pick a frame and send everything back, a few weeks later finished glasses show up.
