It’s actually a lighter projectile. The slap projectile is actually halve the weight of a standard ball round @ about 350 gr. This allows 4000 fps as opposed to the standard 660 gr. ball round @ 3000 fpsFast powder maybe? Heavy projectile? Maybe a combination of the two. You need to match the burn rate to the bullet weight (and case, but that is usually the variable that has the least impact). If all the powder burns before the bullet can start moving down the barrel (increasing the volume and mitigating the pressure rise) the pressure will get high enough to start breaking stuff.
The rifle design could be better. This is how they will all fail with an extremely overcharged round. The thinking shouldn't be "it'll never see rounds that hot." It should be, "If by some freak occurrence, the guns sees a round this hot, how can I arrange the failure points so that the shooter is least likely to get chunks of steel flying into his head and torso." No, I don't think Serbu bears any responsibility - the ammo was clearly well outside of any reasonably predicable envelope.

Though I’m not sure if the other 12 rounds that he shot were reloads. and put the powder it had back in it .. its a full case full of powder ..

My badTrue, but the round he blew the second gun up with was clearly not a SLAP round. Projectile looked like turned brass to me but we don't get a great look.
