I think the only way anyone can get the combination to the safe (without cracking it) is if someone gives it to them. And the key to the secondary lock on top of that. In the Lanza case it is unclear if the weapons were secured in any way on that day he acquired them.
Many people are not security conscious enough when it comes to using a safe and properly securing the codes or access keys. For starters, I would venture that most kids could obtain the code by surreptitiously filming a parent opening it.

Security conscious enough. They need to find a new hobby. They ruin it for everyone who is 100% security conscious. The location and mounting of the safe is just as important as the safe itself. This would prevent any camera angle opportunities, prying etc. For those who would not (at the minimum) smother the combination dial while opening it, (kind of like what you did when you opened your locker in school, there are other options like finger print or other types of electronic recognition.

I can think of no quicker way to start the so called "genocide against trans people" than for them to be identified as likely to become school shooters. Keep it up boys/girls/whatever the hell you claim to be today. Just watch what happens.

According to investigators, a search of Gray’s home uncovered disturbing evidence linking the young boy to the 2018 Parkland shooting at Marjory Stoneman Douglas High School, where 17 people were killed.Following the investigation, the Jackson County officer urged Colt’s father to secure his firearms and keep them locked away. Additionally, authorities recommended that the teen be kept out of school until the matter could be fully resolved, as a precautionary measure.
