You cannot just walk into a lab and ask for a urine test without a medical order.

If you want such testing without a prescription, I suspect that the out-of-pocket cost will be quite high.

And, what is a "12-panel test?" I'm sure it's some kind of toxicology test.

I'd recommend just urinating on the application and sending that back to the examining officer.
