Don't know any of the facts but going out on a limb here and making a prediction whether supported by whatever investigation concludes in future. 1. Gun was pointed in unsafe direction. And 2. Trigger was pulled perhaps more than once. Just saying....

ETA oh and 3. Was not treated as loaded.

More like there was some obstruction in the barrel that the blank propelled out.

The obstruction could have been be an actual bullet, but it's unlikely it was in a live cartridge.

Happened to Brandon Lee from the crow. They wanted to do a scene and didn't want to wait on the prop guys. They made their own round for the frontal scene buy dumping the powder and seating the bullet. They did the scene and fired the gun. They didn't notice that the primer actually pushed the bullet into the barrel. Did the next scene with a blank and it fired the bullet and killed him.

Never underestimate blanks because they're lethal on their own at contact distances. In this case, I agree with spat that there was likely some kind of bore obstruction that the blank blew out. Something like that is what killed Bruce Lee's son when they were filming The Crow.

If it was a barrel obstruction it might not have been a single projectile.

Someone got hit with the main piece, someone else got hit with a fragment.

It was the director and cinematographer. They could have been filming something where he was firing at the camera they were standing behind.

Could have been a single projectile and they were both standing in line behind the camera too, and he got an unintentional Quigley.

When filming a movie, they have to break the rules all the time. They just accept the risk and attempt to mitigate it.

Just like stunt men do things all the time that would have OSHA shutting down the job site if it was any other industry.

When you see them running along a roof, it's not like they have safety railings that they CGI out later or anything.

Most likely this. I read up again on how Brandon lee died on set from a bullet that was unknowingly lodged in a barrel and subsequently propelled by a blank.

I strongly suspect the guy(s) in charge of the gun here have most of not all the blame on their negligence.

Just from the first read on his story there's a lot of negligence to go around. Prop gun or not, he had no business pointing it at people. AB is a privileged idiot. Of course whoever loaded the gun with real bullets should be charged with negligence at the very least. It will be very interesting to see how this goes.
