Insee a lot of variations in handguard fit.
Most of the time it is simple stuff like the angle is slightly off on the taper on the pic-rail. So the rail looks fine until it butts up against the upper.
Not for nothing, but picatinny rails are not milspec. Orginal m16/ar15 never had picatiny rails. So adding one to the upper was one thing.....
I use seekins. Odin works. Atx armory, battel arms development and imported stuff. Most line up better than thia rail......but we are only talking about a few thousands. And i bet the sights will work just fine.
Get a seekins knox rail or an o2 lite from odin. You will teally like em. They are lighter too....itll put you back under 6lbs.

That alignment is particularly poor, yes. It would bug me a bit.

That might bother me for a while but seems you wont be having one scope ring on the receiver and another on the hand guard I wouldn't worry about it. If it were mine I'd camo it and chances are it wouldn't stick out much at all. As long as everything is tight and right the difference in height isn't a problem. Even sights can be adjusted to compensate.
What about this little wedge piece?

I don't think that wedge will matter in this situation. The inner circumference of the hand guard looks like it's already tight to the barrel nut. Now if the rail was higher than the receiver I could see that as a possible issue.

Whats the name/model number of the rail, besides just centurion. On some rails this can be caused by the barrel nut or basically the installer. Lots of rails mount to the barrel nut and if the nut isnt perfectly aligned neither will the rail be when installed. Reason is because some rails mount to the splines on the barrel nut. Does the rail come with its own barrel nut?

It's a little square piece with a u notch in it to straddle the gas tube. Might be in upside down, just a guess. I have never seen a rail that off even the cheapest crap I've seen.

did some looking around, seems the rail uses a perfectly cylindrical barrel nut, so its the picatinny that's off on either the recceiver or rail.
It is a 13" CMR M Lok version rail, this is the barrel nut.

Something is not right with the little tab wedge thing. It could be affecting the way the rail is clamping down on the barrel nut.

File them off. I've done a few so they fit on billet uppers.

It's weird actually, only some of them have that tab thingy.
The Mlok and keymod versions seem to have them, the proprietary screw on a pic rail section version seems not to.

are you able to pull off the rail atm? If i can see what that tab is aliogned with i might e able to give you a bettter answer.
It is not in my possession, no. Nor will it be for a few days/weeks. I am approximately separated from it by 1200 miles at the moment.
That is not a billet upper. What is the tab for/doing?
